MAYF’s 2014 Football and Cheering season!
Early and guaranteed registration is available April 12th through May 30th.
After June 1st late registration is available for $195, and subject to availability.
There is a $100 discount for each registrant after the second registration.
To complete registration the following must be received, participants will not receive their equipment or uniforms until registration is complete!:
- Arrange for payment of the registration fees before the first practice. — This WILL be enforced this season. Please ask any board member for a scholarship request form if you are unable to afford the registration fee.
- Your equipment from 2013 MUST be turned in to complete your 2014 registration.
- The Summary of Releases form.
- An original birth certificate with a raised seal (if one is not on file already).
- Your ORIGINAL year end report card from school (June, 2014).
- A Doctor’s Release Form dated on or after January 1st, 2014.
- Additional forms are required for coaching/volunteer applications. Coaching applications received after June 1st may not be processed in time for the season.
Note: We will be conducting a “live” 2014 registration and 2013 equipment return on Saturday May 17th from 10am-2pm, at the Boys and Girls Club on Pleasant St.
